All WELS VEBA health plan members (i.e. enrolled in any of the four plan options) will automatically be covered under the VSP program at no additional premium cost. WELS VEBA health plan members do not need to separately enroll in the VSP program.
Provider Network
You may use any vision provider you wish; however, vision benefits are greater when you utilize a vision provider within the VSP network. To find VSP network providers in your area, go to www.vsp.com and click “Find a VSP Doctor.” You can enter your ZIP code and find VSP network providers in your area.
Please note that the VSP network includes retail chain affiliates such as Costco and VisionWorks. Wal-Mart and Sam’s Club, although not included in VSP’s network of retail chains, have arrangements with VSP to provide member benefit reimbursements automatically at the point of sale.

Reimbursement Form
If your vision provider does not participate in the VSP network, you may submit a claim to VSP to receive benefits for covered expenses using a VSP member submitted reimbursement form.

When making an appointment with a VSP network provider, simply tell the provider that you are a VSP member before receiving services or hardware. No ID card is necessary.
